Please note there are two clinic locations. Mondays in Silverlake & Thursdays and Fridays in Atwater.

I am currently in-network with Cigna PPO and HMO, and out-of-network with all major insurance providers. If you would like to check your insurance benefits, please email me with your insurance card and birth date at least 72 hrs prior to your first visit. I accept all major credit cards, Zelle, and HSA/FSA. You are responsible for payment at time of service.
